On September 8, 2015, Community Catalysts of California, Inc. learned that an employee’s residence had been burglarized. Among the stolen items was a flash drive that may have contained client information.
Upon review, we have determined that the flash drive may have included the name, address, diagnosis, date of birth, age, gender and/or telephone number for certain current and former clients. No driver’s license, state identification, health insurance or financial account numbers were exposed.
We take the privacy and security of the information in our possession very seriously and we deeply regret these circumstances and are committed to keeping impacted individuals informed.
We have mailed a notification letter to each individual potentially impacted by this event. The letter explained the details of this event and provided steps that can be taken to protect against identity theft, as well a toll-free number to assist those that have questions.
We value the privacy and security of the information in our control. We have taken steps to prevent this type of event from happening again, including retraining our employees on using encrypted device, as required by our company policy. We are also reviewing our data retention practices to ensure that we are not retaining any documents longer than necessary in order to provide services to our clients.
